Student Insurance- if a student isn't covered with a family insurance policy student insurance must be purchased in order for the student to participate on a sport team ( *High School Football insurance must be purchased if student is participating in Fooball)
AIA Brainbook - All Athletes mus complete this to be eligible to play a high school sport (only needs to be completed once during high school)
Form 520- For Transfer Students (this link takes you to the AIA website to complete an online form)
Form 530- For Transfer students with no change in domicile (pdf form)

Rules and Regulations for Gilbert Public Schools Athletics
State and school academic requirements: The Extracurricular Activity Eligibility Policy, JJJ, shall apply to all students who choose to participate in the District's interscholastic programs.
  1. Students must meet the state requirements of passing all classes. An "F" grade. An "I" or a "U" grade will constitute a failure to pass a course.
  2. Students must meet the District requirement of a 2.0 G.P.A. carry over from the previous grading period.
  3. Students must have a physical examination completed per the Gilbert Public Schools Athletic Clearance Packet.
